One Time Payment Authorization 

To schedule your payment to be processed by check, debit card, or charged to your Visa, Mastercard, American Express, or Discover complete the below form and submit the information to our office. Check processing is free however, additional fees apply using a debit card or credit card. Debit card fee is 1% or the amount being processed and 2.9% for credit cards.

By submitting this form, you authorize Howard Morris Group, LLC to debit your account by check, credit card or debit card. A receipt for each payment will be sent to the provided email and the charge will appear on your bank or credit card statement. You agree that no prior notification will be provided unless the date or the amount changes, in which case you will receive a notice at least 10-days prior to the payment being collected. This is permission for a single transaction only, and does not provide authorization for any additional unrelated debits or credits to your account.
